Do not send symptoms, diagnoses, medical records, insurance details, or other private health information through the inquiry form or general email.

Call the office to ask how to send questions about your care. Booking takes place on Zocdoc after you follow a booking link.

The website is designed for hosting on Cloudflare Pages. Hosting and security services can process technical information such as an IP address, requested page, browser information, and request timing to deliver and protect the site. This website uses Google Tag Manager, which can load Google measurement tools such as Google Analytics to show how visitors use the site. These tools can collect technical information such as the pages you visit, your browser and device type, and how you reached the site.

Google reviews, appointment booking, supplement orders, and membership payments open on external websites only after you follow their links. This website does not load an embedded Google map or reviews widget.

4. How Information Is Used

General inquiry information is used to respond to your question and handle related office communications. Technical information supports website delivery, security, and troubleshooting. Clinical care and appointment information are handled through the practice's patient systems and applicable patient procedures.

5. Service Providers and External Websites

Cloudflare provides website hosting. Web3Forms delivers general inquiry form submissions to the office. Google provides Google Tag Manager and the measurement tools it loads. Google hosts the external review profile. Zocdoc handles appointment booking, Athena provides access to the separate patient portal, Fullscript handles supplement orders and supplement lists, and Square handles membership payments. Information you enter there is handled under the notices and terms presented by those services and the practice. This public website does not collect the information you submit in those booking, patient portal, supplement, or payment forms.

This website has no features for selling personal data, building groups of people to target with ads, or targeting ads across other websites. A link to another service does not make its privacy practices identical to those described here.

6. Security and Retention

Use the designated patient channels for sensitive information. No website, email system, or transmission method can guarantee complete security. How long information is kept can depend on the communication, storage system, and applicable recordkeeping requirements. Contact the practice with a question about a particular record or communication. No fixed deletion period is promised by this website policy.
